About the Opportunity

St Carthage's Community Care has an exciting (0.9 FTE) opportunity for a Support Planner to join their dedicated Aged Care Team in Lismore, NSW.

Reporting to the Community Services Manager, you will be responsible for the delivery of integrated, person-centred services that are flexible, innovative, and consistent with the objectives identified in the strategic plan.

You will also contribute to the organisation's strategic plan through the delivery of quality and innovative client services and liaise with a variety of internal and external service providers/agencies in relation to supporting people in the community.

More specifically, some of your key duties will include (but not be limited to):

  • Collaborating with clients and their care partners/stakeholders to identify individual goals and outcomes;
  • Developing, implementing, reviewing, and evaluating person-centred care plans, identifying changing needs and updating as required;
  • Working with clients with complex care needed and people experiencing conflict;
  • Facilitating activities/events that promote opportunities to address the three plagues of loneliness, helplessness, and boredom;
  • Advocating on behalf of individual clients and groups;
  • Encouraging participation of clients from a range of socioeconomic and cultural backgrounds within the community; and
  • Providing professional support, supervision, and coaching to Community Care Workers.

About You

To be successful in this role, you will need to hold a tertiary qualification in Community Services, or similar, along with demonstrated experience in this sector. Due to the nature of the role, you'll need to have an understanding of the relevant Equal Employment Opportunity and Work Health and Safety requirements.

As you will be providing supervision and support to Community Care Workers, it's essential you have strong people management skills and the ability to coach and mentor others. Exceptional written and verbal communication skills will be crucial. It is also paramount that you have a flexible and adaptable nature and creative problem-solving skills.

Computer literacy in a Windows environment is also required, particularly in word processing, spreadsheets, desktop publishing and the use of databases. 

Importantly, St Carthage's is looking for someone with a desire to act as an advocate for clients, their families and their carers. You will be passionate about encouraging older people to think laterally and creatively and be driven to make a difference to their overall wellbeing.

Please Note: This role is subject to a Criminal Record Check and a Working with Children check prior to employment. A current Driver's License and a reliable motor vehicle that is comprehensively insured, and First Aid Certificate, are all requirements of the position.
