About the Opportunity

Sunrise Health Service now has an important opportunity for a credentialed Diabetes Educator to join their population health team on a full-time basis.

Reporting to the Dietitian/Nutritionist Cluster Leader, this is largely a clinical role that will see you providing high quality and individualised diabetes management/care for patients and their families suffering from, or at risk of diabetes and associated conditions. 

The role is part of a team of Preventative and Chronic Disease staff including dieticians, exercise physiologists and health promotion officers. All team members travel regularly to the nine remote clinic sites, all located in remote communities in the eastern Katherine region of the NT. 

Your day-to-day duties will include but not be limited to: 

  • Working collaboratively with all health centre and specialist staff to ensure appropriate planning and implementation of care for patients receiving care for their diabetes and associated conditions;
  • Providing clinical leadership to other health professionals including Remote Area Nurses (RANs), Aboriginal Health Practitioners, medical and allied health professional staff in the area of diabetes management;
  • Managing relevant Medical Benefits Scheme claims for care provided as required and applicable; and
  • Promoting a multidisciplinary team approach to decision making and care provided.

About You

This is a very busy and demanding role, but one that gives you a rewarding opportunity to make a tangible difference to the lives of patients and their families in the improvement of their long-term health management.

To be considered for this role you must be a Registered Nurse or Nurse Practitioner that either holds, or is eligible for, registration with AHPRA. You must also hold existing accreditation as a Diabetes Educator and demonstrated professional experience in diabetes and chronic disease management.

This role would be well-suited to someone who is very strong in their clinical service delivery; has well-developed problem solving skills; and demonstrated knowledge of comprehensive Primary Health Care Principles, including social determinants. Importantly, you will be comfortable living and working in a remote location and have the ability to practice in a sensitive and culturally safe manner in an Aboriginal community setting.

Please note: A current open, manual Driver's License is a requirement of this position. Confirmation of employment is dependent on the outcome of a Criminal History Check and successful application for an Ochre Card.
