About the Opportunity
Yura Yungi Medical Service Aboriginal Corporation (YYMS) is currently looking for an experienced Registered Nurse to join their team as a Chronic Disease Educator. This is a full-time role, based mainly in their Halls Creek clinic, with some outreach to the surrounding community and semi-frequent travel to their satellite clinic, typically for around a week of each month.
Reporting to the Health Operations Manager, you will be providing comprehensive assessments to facilitate the early detection of chronic disease, working in collaboration with the local primary healthcare team to initiate preventative strategies, deliver health education, and promote self-management principals for healthy lifestyles and wellbeing.
Care will be personalised and geared to individual needs, and you will spend time with patients to explore their understanding of their condition, treatments, and their role in their health and wellbeing outcomes.
More specifically, you will be:
- Providing clinical service to clients within generic or specific chronic disease areas, including assessments, screening, interventions, follow up and evaluation;
- Providing clinical leadership to medical and allied health staff to ensure clinical care is best practice, with written follow ups, sound goal setting and care planning;
- Liaising with relevant clinical staff and other service providers to ensure clients families are supported to engage with other internal and external services as required;
- Assisting with training and mentorship of staff where necessary, particularly to empower and upskill Aboriginal Health Workers and other staff; and
- Identifying problems to be addressed in client care plans, including general understanding of their chronic disease, their lifestyle, testing/health check needs or difficulties with medication compliance.
About You
To be considered for this position you will be recognised by AHPRA as a Registered Nurse, with demonstrated post-graduate experience related to chronic condition management, including diabetes, renal disease, cardiac disease and chronic respiratory conditions.
You will have the strong interpersonal and cross-cultural communication skills necessary to promote health literacy and support the self-management of conditions, able and willing to effectively explain the purpose of any investigations, tests or treatments to patients, to ensure that they are fully informed, understand their condition, and are given a voice.
You will have demonstrated experience working in remote communities, with a strong understanding of health issues in these areas, personal strategies for managing stress and the impact they have on health and wellbeing.
You will need to be able to work both collaboratively, as part of a multidisciplinary team, with the high-level writing skills and computer proficiency required to develop and evaluate detailed care plans and prepare reports for management.
Please note: The successful candidate will require a current 'C' class drivers licence, and either hold or be willing to obtain National Police Clearance and Working with Children clearance prior to commencement.
