About the Organisation

New Horizons is a progressive not-for-profit community based organisation that has been operating since 1981.

The organisation provides services in supported accommodation, supported employment and aged care for over 2,200 people. New Horizon's non-profit commercial packaging service specialises in providing solutions for commercial clients while providing employment for people with a disability.

The organisation's head office is in North Ryde, however services are provided in the Sydney, Central Coast, Hunter, Mid North Coast, Blue Mountains and Northern Rivers regions. New Horizons is growing rapidly, with approximately 450 employees in NSW.

About the Opportunity

New Horizons has exciting opportunities for a full time Case Manager in North Ryde and a part time Case Manager on the Central Coast.

The main aim of the Case Manager roles is to assist and enable people with a disability and/or mental illness to maximise their independence and participation in the community by increasing access to local services and provide support and living skills training. 

While each Service Recipient's daily requirements will depend on their individual circumstances, your day to day duties will fall within the following broad areas:

  • Coordinating the delivery of services to Service Recipients;
  • Demonstrating a commitment to personal and professional development;
  • Developing and maintaining collaborative relationships with colleagues and others; and
  • Working within the Occupational Health & Safety policy and protocols of New Horizons

The ideal candidate will possess a relevant tertiary qualification, as well as demonstrated skills in case management, service coordination and staff supervision. The applicant for the Central Coast will have experience working in the field of mental illness whereas the North Ryde applicant will require experience with intellectual disability.

You should be an empathetic individual, with strong communication and organisational skills, and the ability to work in challenging situations. An enthusiasm for the industry, and a genuine desire to help others and improve the community, is essential! 

You will need a current NSW driver's licence and own vehicle.

You will be rewarded with an attractive remuneration package circa $50,000 - $55,000 (pro rata), dependent on experience, plus a car allowance!
