About the Opportunity
If you're truly client focused and believe in clients' abilities to reach their potential, exciting opportunities have become available within Annie Kenney’s Young Women’s Refuge, part of Centacare Tasmania .
A rewarding opportunity exists for 3 experienced Case Managers (PT) and 2 Case Managers/ Support Workers (Casual) to join the service which provides emergency accommodation support and assistance to young women aged 13-20 years who are currently homeless or at risk of homelessness.
Reporting to the Team Leader, some of your daily contributions will include:
- Bio-psycho-social assessment, including development of a comprehensive Case Management Plan with the client, articulating the client’s goals and individually tailored interventions;
- Forming linkages with training, employment and other social support services;
- Supporting clients transition to social housing tenancy under the Same House Different Landlord program, private tenancy or home ownership;
- Undertaking pre and post-measurement of health and wellbeing outcomes for clients focused on three areas of the client’s life: trauma recovery; mental and physical health; and social inclusion (housing, independent living skills, support networks, employment and training); and
- Participating in fortnightly clinical and operational supervision.
The successful applicants will hold tertiary qualifications in a relevant discipline (such as Psychology, Social Work, or Diploma in Social and Community Work) with experience in working with high and complex needs clients. Applicants will be expected to work within a practical and therapeutic case management context with capacity to implement a range of theories and models of practice.
These roles require highly organised candidates with experience in providing case work, including all associated reporting, and conducting bio – psycho-social assessments. Within each day you will be communicating with a diverse range of people, both staff and clients, so you must be a highly effective communicator, in written, verbal and interpersonal capacities. If you excel in problem solving, multi-tasking, and finding creative solutions this role will suit you.
The successful applicants must be available to work within a set rotating roster which includes sleepovers, evening and weekend shifts.
A current driver's licence is required. Please note you must also have Australian Citizenship, Residency or Work Visa to apply for these roles. Shortlisted candidates will be subject to an employee screening process including a national police check.
