About the Organisation
The Coonamble Aboriginal Health Service (CAHS) aims to improve Aboriginal people's access to primary health care through the provision of relevant programs and services within a dedicated service.
The Coonamble clinic opened in August 2007, servicing an area measuring 15,000 sq km including Coonamble, Gulargambone and Quambone. Staff consists of 12 health staff and two full time GPs, with a mix of cultural background within the organisation.

About the Opportunity
Coonamble Aboriginal Health Service (CAHS) has an exciting full time opportunity available for a skilled Registered Nurse as a Chronic Care Coordinator to join their team in Coonamble, NSW.
Working closely with the local Aboriginal community, this pivotal role will be responsible for the overall coordination of CAHS's Chronic Care Enhancement Program, which aims to provide specialised services to improve the health, well-being and independence of clients with a range of chronic care needs. Through your leadership of this program, you'll be improving the day-to-day care of clients with conditions like diabetes, chronic obstructive pulmonary disease, chronic heart failure, coronary artery disease and hypertension.
Your responsibilities will include developing appropriate support mechanisms for chronic care staff, service networks, monitoring health outcomes and ensuring performance indicators are met.
More specifically your duties will include (but will not be limited to):
- Providing program leadership, management and support for the service development initiatives;
- Working closely with patients, carers and the primary care team, and providing active contribution to the development of patient’s individualised care plan;
- Developing and implementing community engagement activities to promote Chronic Care;
- Improving systems coordination and communication between service providers; and
- Improving the coordination of available services including timely and affordable access to culturally appropriate allied health, specialist and transport services for Aboriginal people with complex conditions.
To succeed in this role, you'll be a Registered Nurse with practical in-the-field clinical nursing leadership experience, preferably within a primary health care setting. You'll need a proven understanding of chronic care issues and a commitment to evidence based practice. It would be beneficial to have health promotion/education experience in your background, as well as experience in the area of Aboriginal health.
This role will suit a strategic and collaborative leader with demonstrated skills in healthcare project management (planning, data analysis, reporting), and solid computer literacy. Your background within a multi-disciplinary healthcare team will be supported by excellent written and verbal communication skills, the ability to develop rapport with a wide range of stakeholders, and an understanding of EEO and OH&S principles. All applicants need a current driver's licence.
