About the Opportunity

Orange Aboriginal Medical Service (OAMS) has an exciting opportunity for a Registered Nurse to join their dedicated and supportive team as Chronic Care Coordinator in Orange.

Reporting to the Practice Manager as part of the Aboriginal Chronic Disease program, you'll assist in improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care. Your work will help to improve access to and linkages between essential primary and secondary health services for your patients.

Using your nursing experience and patient-focused approach, this is an opportunity to work alongside local GPs in providing hands-on and proactive management to Indigenous patients.

Key responsibilities of the role include (but are not limited to):

  • Working closely with General Practitioners and Practice staff to support improved care planning and continuity of care across service boundaries;
  • Working closely with the patient, carer and primary care team, provide active contribution to the development of patient’s individualised care plan;
  • Sharing responsibility with the patient and their family for the implementation of care plans aimed at improving health and social outcomes for people with chronic complex and continuing care needs;
  • Ensuring all information given is provided in a way that the patient, carer and community understand;
  • Providing early intervention, coordinated care and information and referral services; and
  • Improve follow-up care for patients with a chronic disease and the identification of support needs of patients.

To be successful in this role, you will be a Registered Nurse with significant post-graduate clinical nursing experience, and a commitment to providing superior patient care in a community nursing environment. You will have the demonstrated ability to Triage and manage patient care in a busy clinical setting, and will be dedicated to earning credibility and trust from the local community.

It is expected you will have previous experience working effectively with vulnerable families within a family centred framework. Additionally, you will require sound knowledge and contemporary understanding of Aboriginal social and emotional issues. Your background within a multi-disciplinary healthcare team will be supported by excellent written and oral communication skills, the ability to develop rapport with a wide range of patients, and an understanding of EEO and OH&S principles. All applicants need a current driver's licence.

Given that OAMS is only 5 years old, you will be able to make your mark on the organisation as your career grows in line with the expanding service.
