About the SAILS Program

The Strength Advocacy Independence Life Skills (SAILS) Program is a mental health outreach program that provides individual support to consumers both men and women between the ages of 18 - 65 years experiencing the effects of a mental health condition, who live in the ACT. The program is funded through the ACT Government Health Directorate, Division of Metal Health, Justice Health and Alcohol and Drug Services.

SAILS focuses on supporting the individual to achieve their chosen goals, with a focus on resilience and a belief in the rights and responsibilities of an individual. The programs focus is on the development of the consumers needs and a focus on the strength of a person.

About the Opportunity

CatholicCare Canberra & Goulburn has a Part Time opportunity for an enthusiastic Mental Health Outreach Worker to join their team, based in Watson, Canberra.

Working as part of the SAILS Program, you'll be providing assessment, intervention and discharge planning for adults with a mental illness who are consumers of the program. Your individual duties for each consumer will be determined based on a case management / person-centred model of service delivery.

Hours will be flexible, as you work a minimum of 7 days per fortnight. You'll be working from the campus of the Australian Catholic Univervity site, with access to all facilities.

More specifically, your duties will include:

  • Assisting the consumer to promote resilience and maintain mental wellbeing, reducing the incidence of crises and hospitalisations;
  • Supporting consumers to access and maintain suitable housing, and develop links with and access health, education, employment, welfare and other services;
  • Providing access to advocacy and information services;
  • Addressing the consumer’s risk factors, and facilitating positive relationships with family carers, significant others and social contacts;
  • Encouraging consumers to maintain participation in education, training or employment;
  • Promoting daily living skills such as hygiene, food preparation and banking; and
  • Liaising and networking with colleagues and Mental Health Workers from other organisations.

To succeed in this role you'll be a flexible team player with experience in respectfully working with adults with a mental illness. You'll hold tertiary qualifications in a relevant health / welfare field, and will have a proven background in conducting detailed assessments of clients. You'll need experience working within a case management framework to develop recovery plans, and teaching living skills which improve your consumers' independence and wellbeing.

You'll be a patient and enthusiastic individual who understands the importance of managing time effectively to meet deadlines and is able to effectively handle high pressure situations. You'll have existing knowledge of relevant agencies, resources and supports available to adults with a mental illness in the ACT, and how to link consumers to them.

All applicants must hold a current driver's licence and be willing to undertake a Criminal History Check before commencement.
