About the Opportunity

Due to increased funding, St Carthage's Community Care currently has an exciting opportunity for an Assistant Coordinator to join their dedicated team in working throughout the Northern Rivers region of NSW. The major focus of this position is to promote opportunities to address the three plagues of loneliness, helplessness and boredom.

Reporting to the Coordinator, you'll be responsible for creating human habitats and a pathway to a life worth living.  This will include facilitating easy access to human and animal companionship, creating opportunities to give as well as receive care and creating environment in which unpredictable interactions and happenings can take place.  

As a proactive member of the Aged Services team, you will also be providing day to day supervision to community care workers as well as implementing, reviewing and evaluating community care packages for older persons.

As you will be liaising with other services and external service agencies and providers, it is essential to have a working knowledge of the relevant program standards, guidelines, target groups and special needs groups and the capacity to contribute to the strategic and business planning process.

Some of your specific daily duties will include (but not be limited to):

  • Identify and develop client goals through assessment processes;
  • Plan, develop, implement and evaluate individual, community care packages and person centred care plans;
  • Actively promote and advertise St Carthage's Community Care to clients, families and other relevant stakeholders;
  • Maintain accurate records and monitor the use and storage of equipment, resources and materials;
  • Ensure Community Care Workers are aware of their role and participate in induction and orientation for new staff as required;
  • Assist the Coordinator in undertaking research to identify the needs/issues in relation to target groups; and
  • Develop professional relationships with internal and external stake holders.

To be successful in this role you'll be committed to the Eden Alternative and will have significant demonstrated experience in the assessment, planning, development, implement and evaluation of Home Care Package Services and HACC. Importantly, you'll have the ability to work with clients with complex care needs as well as people experiencing conflict.  

You will need to be computer literate in a Windows environment, with the ability to work both in a team and autonomously and the ability to make immediate decisions when required. Effective time management is essential, as is the ability to provide regular structured supervision, coaching and debriefing support.
