About the Organisation

In the past 28 years since its inception in 1986, Carnarvon Medical Service Aboriginal Corporation (CMSAC) is an Aboriginal Community Controlled Health Service (ACCHS) that has aimed to provide holistic primary and secondary health and medical services to the Aboriginal people of Carnarvon and the surrounding areas.

CMSAC offers a great immunisation program, along with podiatry and dental services. Their HACC and Stronger Families Stronger Culture programs are well developed.

About the Opportunity

CMSAC has a fantastic opportunity for an empathetic Home & Community Care (HACC) Coordinator to join their friendly team based in Carnarvon, WA.

Reporting to the CEO, you will be responsible for the overall management of the Home and Community Care project. In this role, you will enable improved outcomes for people who are frail aged and those who have a disability, by providing them with quality care appropriate to their needs.

More specifically, some of your duties will include (but will not be limited to):

  • Ensuring that HACC staff fulfil the requirements of their job descriptions;
  • Organising and maintaining an effective volunteer program, including registration and training as required;
  • Developing and implementing care plans for clients eligible for HACC services;
  • Prepare funding and policy submissions to Government and other bodies in consultation with CMSAC’s General Manager; and
  • Developing and implementing appropriate programs and policies for meeting the needs of the local Indigenous clients.

To be successful for this role, you will have the demonstrated ability to manage a human services organisation, including developed staff management, administration and financial management skills. You'll ideally have relevant professional qualifications in nursing, occupational therapy, social welfare or an equivalent field. The ability to assess medical, social and environmental factors, and in particular, the degree of functional disability of clients will also be advantageous to this role.

A detailed understanding of, and commitment to, HACC principles and related programs, along with an understanding of the issues affecting the frail aged and younger people with disabilities is essential. You'll also require a working knowledge of relevant community resources. Experience in direct service delivery work with the HACC client group is desirable, though not essential.

The successful applicant will be a dedicated, client-focused individual with experience working within the community and the ability to work with a Management Committee. Your excellent communication skills will provide you with the ability to liaise, negotiate and resolve conflicts effectively.

A current 'C' class driving licence is essential, as is a Police Clearance.
