About the Opportunity

Inner East Primary Care Partnership is seeking a dedicated Program Coordinator - Services and Integrated Chronic Disease Management to join their multidisciplinary team. The role will be responsible for leading systems change and improving service integration between member organisations, and supporting agencies to implement best practice care and service system improvements that enhance chronic disease management, community aged care and client centered care.

Key responsibilities of this role will be to:

  • Build positive relationships with stakeholders across the catchment;
  • Improve catchment wide approaches to chronic disease management and community aged care through research, collaboration, coordination and dissemination of information and promotion of best practice approaches;
  • Participate in state-wide networks and forums for Integrated Chronic Disease Management and Service Coordination;
  • Support capacity building across the catchment with a focus on care, collaboration and coordination of services;
  • Initiate, lead or participate in projects that enhance coordinator of care and chronic disease management;
  • Resource and support relevant committees, forums and networks to facilitate improved communication; and
  • Identify opportunities to coordinate responses to policy and direction papers in consultation with the Executive Officer and relevant stakeholders.

IEPCP is looking for an energetic and innovative person with relevant tertiary qualifications in a health related field and demonstrated experience in building strong relationships among key stakeholders. An understanding of the primary health and community services sectors is vital including, an understanding of service pathways and the health needs of older people and those with chronic illnesses. Demonstrated skills and experience in the planning, delivery and evaluation of services, programs and projects is required. 

The successful applicant will be able to see how their day to day work fits into the ‘bigger picture’ of addressing systems change and service coordination in improving catchment wide approaches to chronic disease management and community aged care.

You will have excellent interpersonal and communication skills (including effective written communication), and be able to work independently as well as part of a small team. You will have excellent organisational and time management skills.
