About the Post Acute Care Program

This position sits within the Health Independence Program (HIP) suite of services that are delivered across St Vincent’s Health Melbourne (SVHM) and North Richmond Community Health (NRCH) .  Post Acute Care is funded by Department of Health and Human Services and assists clients to recuperate at home following discharge from a public hospital.

About the Opportunity 

NRCH has an exciting opportunity for a registered Physiotherapist or Occupational Therapist to take on the role of Care Coordinator and join their professional Post Acute Care (PAC) program, based in Richmond.

The Care Coordinator is the primary contact for clients, assisting them navigate the service system. PAC has a flexible service delivery model enabling the program to provide a responsive service which includes brokerage of a wide range of health and community services and provision of clinical care by program staff when necessary.

More specifically, some of your responsibilities will include:

  • Developing care plans with clients as well as procure, coordinate and monitor post acute services;
  • Maintaining electronic client records (using TCM) for clients under care, including demographic data, interventions and outcomes, for the purpose of meeting reporting requirements, quality improvement and program planning;
  • Providing home and/or centre based clinical services to clients referred to the program;
  • Developing and maintaining strong networks with other Health Independence Programs, hospital clinicians and community service providers;
  • Participating in organisational, PAC and HIP quality improvement activities;
  • Making a positive contribution to the functioning of the role and the team through active participation in training, team meetings and service initiatives;
  • Identifying professional development objectives and participate in performance review;
  • Promptly report adverse clinical and service related situations that may place the client, the program or North Richmond Community Health at risk;
  • Complying with privacy and confidentiality policies at all times; and
  • Complying with Department of Health and Human Services HIP program requirements.

The successful applicant will be a registered Physiotherapist or Occupational Therapist with significant experience working within the health and/or community services sector. You will have experience working with people with complex issues in a health setting (including homelessness, drug & alcohol, mental health, dementia, refugee and migrant communities), and the proven ability to work with people from CALD (culturally and linguistically diverse) backgrounds.

NRCH is looking for an enthusiastic, highly organised team player with a strong knowledge of the community services available to clients in the recuperative phase. To ensure the smooth rollout of services to clients, you will have demonstrated ability to identify issues, problem solve, be an effective communicator with a wide range of stakeholders and meet deadlines in a fast-paced environment.

You must have excellent computer skills including MS Office suite. All applicants require a current driver's licence.

The ability to speak a second language is highly desirable.
