They provide high quality, culturally and linguistically appropriate aged and community care that responds to people's changing needs. Their services include home care packages, domestic assistance, respite and transport throughout the Inner West, South West and South East of Sydney. 

About the Opportunity

Multicultural Aged Care Inc. has an exciting opportunity for an experienced Home Support Program Coordinator to join their dedicated team in Sydney, NSW. This newly created role is offered on a part-time basis with the flexibility to choose your own hours (Tuesdays must be worked in order to attend staff meetings).

Reporting to the CEO, this role will see you responsible for the effective operation and coordination of the Home Support Program (HSP). Building on your well-established network in the sector, you will facilitate and provide community outreach, driving an increase in referrals for the HSP and HACC (Home and Community Care) programs.

More specifically, some of your key duties will include (but not be limited to):

  • Coordinating and delivering the HSP;
  • Driving referrals for the HSP and HACC programs through community outreach;
  • Providing comprehensive, holistic assessment of clients;
  • Maintaining appropriate client and carer contact to ensure that their needs are regularly reviewed;
  • Developing, evaluating and revising client care and support plans;
  • Monitoring clients' needs, health and well-being by face-to-face and phone support;
  • Promoting the service through networking and liaising with community groups; and
  • Overseeing maintenance of client and program records to ensure accountability to funding bodies.

To be successful in this role you'll have demonstrated experience in the assessment, planning, development, implement and evaluation of HACC services, particularly in a CALD community. Alternatively, candidates with transferable skills and experience as well as an understanding of HACC programs will also be considered. Applicants will have relevant tertiary qualifications (e.g.- Aged/Home Care) or equivalent work experience. Importantly, you'll have the ability to work with elderly clients with low level care needs.

You will need to be computer literate in a Windows environment, with the ability to work both in a team and autonomously and the ability to make immediate decisions when required. Effective time management is essential, as is the ability to effectively negotiate and solve-problems to ensure positive outcomes for clients.

Multicultural Aged Care Inc. is seeking a self-motivated individual with excellent verbal and written communication, conflict resolution and organisational skills who is cooperative and flexible in both team and individual tasks. The ability to work in a multidisciplinary environment and to supervise and support bilingual staff is vital.

While not essential, candidates with an applied knowledge of enabling consumers to achieve independence will be highly regarded. Similarly, experience in the community care sector and established connections with community and providers in the South East Sydney region.

To be successful you must hold, or have the ability to obtain a First Aid Certificate and a National Police Check, as well as a current driver's licence with access to a reliable vehicle.
