About the Opportunity

GOC Care has two excellent opportunities for Case Managers to join their professional teams in South Brisbane and the Gold Coast, QLD.

Reporting to the Program Manager, you will primarily be responsible for coordinating the planning, implementation and ongoing evaluation of client care services and support. This includes assessments, care plans and reviews. 

More specifically, your duties will include (but will not be limited to):

  • Providing effective supervision, monitoring, support and direction to Care Workers;
  • Developing client care plans and reviewing these plans and services to ensure client needs are met;
  • Liaising with other service providers, health professionals and community organisations and groups to ensure a holistic approach to services;
  • Managing client services, support and requirements in compliance with legislation and policies;
  • Ensuring the efficient use of program resources; and
  • Contributing to a team of case managers to ensure a quality client service.

GOC Care is looking for an individual with demonstrated case management experience. Proven knowledge of community services will be highly regarded, with emphasis on experience within aged care and/or culturally and linguistically diverse (CALD) environments.

You will be confident in your administrative and operational skills, as well as your ability to contribute to organisational objectives in a service delivery environment.

Experience in staff supervision, along with exceptional interpersonal and communication skills will be key, as you will be engaging with clients and staff across all levels.

The ability to multi-task, high level organisation skills, and experience in managing competing priorities effectively is also required.
