About the Opportunity

Nunkuwarrin Yunti has an exciting opportunity for a passionate, hard working Aboriginal Health Worker/ Practitioner to join their Primary Care Services Team in their Elizabeth Downs service in South Australia. This position level is subject to professional registration and scope of practice of the incumbent, therefore job and person specification will be adapted accordingly. 

Reporting directly to the Practice Nurse, you'll be responsible for working collaboratively with the Practice Nurse , GP's and other members of the health care team to provide day-to-day clinical care at the Brady Street clinic.  

Some of your key responsibilities will include, but may not be limited to:

  • Working collaboratively with other members of the health team to ensure quality and safety of care and smooth running of clinic processes;
  • Promoting the importance and benefits of general preventative health assessments and immunisation and ensuring access to these services;
  • Undertaking AHW/AHP assessments and follow up care as planned through health assessments, care plans and referrals from other members of the multi-disciplinary team;  
  • Liaising with internal and external stakeholders as necessary;
  • Performing tasks requiring the exercise of judgement, initiative and confidentiality;
  • Maintaining a positive working relationship with others;
  • Modelling behaviours and attitudes that are culturally sensitive in all interactions with staff/clients/stakeholders; 
  • Participating in personal and professional development activities; and
  • Ensuring compliance with a range of administrative practices.

To be considered for this role you will hold a Certificate IV in Aboriginal Primary Health Care or equivalent. You may also have a current Medicare provider number, or the eligibility to obtain one. You'll have demonstrated vocation experience consistent with the role and responsibilities of an Aboriginal Health Worker.

While not essential, registration with AHPRA and/or experience working within a Community Controlled Health Service will be highly regarded. Additionally you'll have experience using computer software such as Patient Information Recall Systems and other databases.

To hit the ground running in this role you'll be supported by a multi-disciplinary team, augmented by a range of visiting specialists and allied health professionals who provides comprehensive and coordinated health care services for the Aboriginal community. Therefore, you'll have the ability to work cohesively with your new team but also the drive to work independently when needed to assess priorities and organise your workload. 

In addition, you'll have well-developed clinical skills and a sound knowledge of best practice approaches to comprehensive primary health care and a commitment to workplace diversity, participation and OH & S. In order to best liaise with internal and external stakeholders you'll also have the ability to resolve conflict, solve problems and negotiate outcomes.

Importantly, you'll be able to demonstrate experience in supporting Aboriginal Health/community workers, and will be driven to improve the health outcomes in Aboriginal and Torres Strait Islander communities. You will have knowledge and understanding of Aboriginal societies and culture, and the ability to communicate effectively and sensitively with Aboriginal people.
