Based in Stawell, approximately 235kms west of Melbourne, Grampians Community Health (GCH) is a not-for profit Community Health Centre that has been providing coordinated health and community services to people within the Central Grampians and Wimmera sub regions since 1986. Their multi-disciplinary approach to psychosocial health aligns with a Social Model of Health philosophy and aims to promote healthy living in the Grampians whilst also improving the standard of living.

About the Opportunity 

GCH has a rewarding opportunity for a Facilitator to join their team in Horsham, VIC. This is a part-time position, ongoing until June, 2019, with extension subject to funding.

As a Facilitator, you’ll be working in the Community Day Rehabilitation Program to provide leadership to the staff within the catchment in order to ensure community access to the alcohol and other drugs support services provided within the program is maintained.

Some of your duties will include, but will not be limited to: 

  • Developing professional working relationships with other providers, including local Aboriginal Cooperatives;
  • Identifying, establishing and managing referral pathways;
  • Working in collaboration with other agencies to meet patient needs;
  • Providing participants and potential participants with information and advice, including education on drug safety;
  • Providing interventions such as cognitive behavioural therapy;
  • Involvement in the local drug action plan; and
  • Providing staff support as required. 

GHC is searching for an individual with a Certificate IV in Alcohol and Other Drugs (AOD) and Mental Health or qualifications in other relevant fields such as social work, psychology, or welfare work. It is a requirement that you have at least 2 years’ experience working within the AOD field, with a strong knowledge of current AOD interventions. Candidates with experience in program coordination, generalist casework, counselling and group work will be highly regarded. An understanding of the social model of health and health promotion will be invaluable to this role.

You’ll be well-equipped to manage various group dynamics, and confident in working as part of a multidisciplinary team. You’ll have proven skills in the application of assessment, collaborative care, and the development of individual care plans. You’ll be an energetic and flexible leader, who is confident in successfully managing change. Above all, you will be committed to improving the health outcomes of patients struggling with various issues concerned with alcohol and other drugs.

Please note: It is a requirement of this role that candidates possess a current driver’s licence, a Working With Children Clearance Check, and are willing to undergo a police check prior to employment.
