About the Opportunity

Due to an exciting growth period, Greenwood Manor now have an opportunity for a Clinical Care Coordinator to join their team based at Dingley, VIC, on a full-time basis.

The Clinical Care Coordinator is primarily responsible for providing clinical and administrative support and to ensure the effective and timely care for all residents, clients and patients. Working closely with the Facility Manager, you will keep abreast of any issues, concerns, or updates regarding staff, residents and/or representatives, or other workplace matters as required, and implement suitable action accordingly.  

Your day-to-day duties will include, (but not be limited to):

  • Liaising with doctors, nurses, other health care providers and the pharmacist to facilitate optimum health care and clinical practice for the residents;
  • Supervising, supporting and providing education and orientation for new staff and existing staff when required;
  • Reviewing Resident Care Plans on a 1-2 monthly basis, or more frequently as required, including family consultation; and
  • Providing direct supervision of staff to ensure they are meeting the care needs of residents and maintaining appropriate documentation to satisfy funding and accreditation requirements.

To be successful in this position you will have extensive experience as a Registered Nurse, preferably in an aged care environment, with a strong understanding of ACFI and Accreditation Standards. You will also need to hold current AHPRA registration. 

As you will be operating in an at times challenging environment, you will need to have a solid understanding of patient care and the ability to work within structured processes. You will possess exceptional organisation and time management skills, which will assist in your ability to perform above and beyond job requirements. 

This role would be well suited to someone with a personable and approachable demeanour, looking for a 'hands on' role and the opportunity to work within a close-knit and caring team, where everyone pitches in and works together to get the job done.
