New Aged Care is a supportive General Practice based in Woodend. About the Opportunity

New Aged Care now has multiple opportunities available for full-time or part-time Registered Nurses (Div 1) to join their team of GP's, Specialists, and nurses based in the western suburbs of Melbourne, VIC.

In this role, you will be primarily responsible for providing - and coordinating - high-quality assessment and clinical care to clients of New Aged Care. 

More specifically your key duties will include (but will not be limited to):

  • Applying best practice principals to clinical care;
  • Delivering an equitable standard of care regardless of clients and colleagues different values, beliefs, culture and social context;
  • Maintaining accurate documentation of patient assessment and goals;
  • Liaising with doctors, primary care givers and other members of the patients' multidisciplinary team in case conferencing care plans for the clients, and coordinating routine rounds;
  • Seeking support from colleagues to identify learning needs; and
  • Exchanging information with community agencies to promote awareness of service delivery, leading to appropriate referrals between agencies.

To be successful in this role, you will be a Division 1 Registered Nurse with a current Practising Certificate. 

While not essential, previous experience in aged care or community nursing will be highly regarded.

The ideal candidate will be an effective communicator, have solid computing skills, and employ excellent clinical assessment and observation skills.

You will thrive working in a team environment, display exceptional organisational skills, and have a high-level of respect for the elderly.

Please note: Some travel between to head office in Woodend will be required once a week. Candidates must have a current driver's licence and access to a reliable vehicle.
