Community Restorative Centre - Changing Lives, Reducing Crime

Founded in 1951, Community Restorative Centre (CRC) is an independent community organisation with a broad funding base, dedicated to supporting people affected by the criminal justice system, particularly prisoners, ex-prisoners, and their families and friends. CRC is the lead provider of specialist throughcare, post-release, and reintegration programs for people transitioning from prison into the community in NSW. All CRC programs aim to reduce crime and break entrenched cycles of disadvantage, offending and imprisonment.

About the Opportunity

With the NDIS becoming fully operational, Community Restorative Centre (CRC) is now looking for an NDIS Transition Coordinator to join their team in Chippendale, NSW. This role is being offered on a contract basis until June 2018.

This newly created position will see you working in partnership with Corrective Services NSW to facilitate the smooth transition for existing Ageing Disability and Home Care (ADHC) clients to the NDIS, prior to their release from prison

You'll collate and document data and information relating to the transition process, to assist in the development of a comprehensive document aimed at providing recommendations to assist the referral process for clients to the NDIS in the future.

In partnership with Corrective Services NSW, some of your key areas of responsibility will include:

  • Identifying which individuals within the corrective system are eligible for NDIS funding;
  • Liaising with service providers and developing supports for individuals exiting custody to increase wellbeing and reduce offending behaviours;
  • Increasing awareness of the NDIA, Planners and service providers of the complex support needs of this target population; and
  • Documenting systemic challenges and making recommendations for improvement in the referral of people with disability in custody to the NDIS.

About You

CRC is looking for a driven individual who is ready to take on an exciting new challenge! You'll thrive in a fast-paced position with lots of autonomy.

The successful candidate will hold relevant qualifications and/or experience directly related to working with people with an intellectual disability and complex needs, including cognitive impairment, mental health and AOD issues. You'll also have a demonstrated understanding of the issues faced by people involved in the criminal justice system. 

Due to the nature of the role, experience in establishing and maintaining relationships with relevant stakeholders is essential and will allow you to advocate and facilitate referral pathways for clients. In addition, you will have experience in, or strong understanding of, working within a trauma informed framework. 

Please Note: A current Driver’s License, current Working With Children Check, and Criminal History Check are all required before commencing this position.
