The Role 

The Case Coordinator (CC) is responsible for providing effective leadership and management of the House Managers. A strong focus on the development of person centred plans designed to meet the needs and enable the goals of each participant.  It is critical that the CC maintain strong relationships with participants, their families, and key stakeholders to ensure the best services are provided in line with individual goals and plans. This role also includes: management and development of staff, administrative duties including service agreements, medication system oversight, critical incident reporting and formal supervision.

What would you do 

The main things you would do include :

  • Oversee the successful implementation and review process of all Individual plans working from a strength based and person-centred approach
  • Assist the Senior Case Coordinator with intake and induction of new participants including creation of service agreements
  • Managing and accessing staff by signing off on the  monitoring the effects and successes of behaviour plans with the individuals who have such a plan in place. This includes the monitoring and reporting of serious incidents and restrictive practices 
  • Assess participants’ needs and plan, develop and implement training, education and support programs to help them meet their goals 
  • Facilitate communication between stakeholders, advocating on behalf of participants, assessing and reviewing individual needs and identifying appropriate interventions that promote best practice
  • Supervision and management of House Managers
  • Feedback the status of staff performance to the Human Resources Manager as part of the performance appraisal process
  • Monitor, review and document the success and/or failing of IPP goal implementation towards specified objectives and modify as necessary to accurately reflect the persons current situation as each assessment takes place
  • Review each participants’ medication administration ensuring practices are recorded accurately in line with Quality and Safeguarding requirements

Exceptional manager providing a high level of leadership and support to a team

Competed Cert IV in Individual Support (Disability) or equivalent 

Strong understanding of Indigenous culture and is able to work within and provide support in a diverse environment 

Strong writing and reporting skills with significant knowledge of the NDIS 

Has a proven track record of working with clients with high behavioural or complex needs
