The Eastern Melbourne PHN (EMPHN) is a new organisation established to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and to improve coordination of care to ensure patients receive the right care in the right place at the right time.

The Partners in Recovery (PIR) project has been developed by the Commonwealth to provide care for the most vulnerable people with severe and persistent mental illness to prevent those with complex needs from “falling through the gaps.” As the lead agency, EMPHN will provide care coordination through Support Facilitators to consumers who live in or receive services in the EMPHN region in collaboration with regional PIR partners, GPs, carers and families. 

We are looking for suitably qualified (Degree in Psychology, Social Work, OT or relevant field) and/or experienced Support Facilitators with a passion and drive to deliver quality services for the PIR cohort.  The Support Facilitators will have experience of working with consumers who have severe and persistent mental health needs and also a range of other complex needs.  We need individuals who can look at the bigger picture and are able to understand the need for a multi disciplinary approach to delivering community based ‘wrap around care’.  This will mean that the successful applicants will need to demonstrate excellent partnership skills and the ability to collaborate with a variety of different professionals.  At the core of PIR is a ‘strengths based’ model and it is therefore vital that support facilitators understand and advocate for this approach when supporting the consumer in their recovery journey.  

These positions will be based across the Outer East across our Knox, Lilydale or Healesville offices and will report to the PIR Manager East.   For more information, please refer to the attached position description.
