About the role

At LDK Seniors’ Living, we are seeking Clinical Case Managers to join our high-performing team! Our ideal candidate would enjoy administrative functions, as this role is predominantly non-clinical. Working from Monday to Friday, our Clinical Case Managers’ key responsibilities include:

Assessment and Care Planning:

  • Manage care for residents, ensuring their physical, emotional, and social needs are met.
  • Implement personalised care plans in collaboration with residents, their families, and other health professionals.
  • Maintain records of care plans and government-funded home care packages, health changes, and any interventions provided.

Resident Engagement:

  • Coordinate with key stakeholders in the organisation to address resident care needs.
  • Determine eligibility and manage residents’ entitlement to government-funded care.
  • Collaborate with the lifestyle team to develop social, community service and activity programs to meet each resident's goals and foster community connectedness.

Stakeholder Engagement:

  • Work closely with doctors and allied health providers to ensure ongoing care for our residents.

To be successful in this position, strong people skills are a necessity. Hands-on clinical duties are relatively infrequent, but you will need to be a very capable communicator.

We will provide full training to ensure the successful candidate can complete the administrative functions of their role successfully and build relationships.
