About Amberfield

About the role

We are seeking a Clinical Case Manager to assist our new residents as they transition into their new homes. This role involves providing our care team members with a comprehensive understanding of the residents' care needs through thorough clinical assessment and care planning.

Role responsibilities:

  • Providing pre-entry clinical assessment, move-in support and care planning.
  • Collaborating with our sales and resident services teams to ensure excellent resident outcomes.
  • Conducting move-in meetings with new Amberfield residents.
  • Liaising with doctors and other allied health professionals where required.
  • Maintaining records of care plans and government-funded home care packages, health changes, and any interventions provided.
  • Determining eligibility and managing residents’ entitlement to government-funded care.
  • Supporting our care team with resident caseloads as the village and team grows.

Qualifications, experience and skills:

  • Current AHPRA registration as a Registered Nurse
  • Experience conducting care assessments and creating care plans
  • Excellent communication and stakeholder engagement skills
  • Relevant clinical experience as an RN (clinical experience in geriatrics would be highly regarded)
