About Greenway Views
LDK Seniors’ Living’s flagship village, Greenway Views, is in Greenway, Tuggeranong in Canberra, opposite the South Point shopping centre and walking distance to the Tuggeranong town centre. The village consists of 327 apartments, spread over five buildings that is about half a kilometre long from end-to-end. Greenway Views is home to nearly 400 residents over the age of 65. The village has over 6,000 square metres of communal space facilitating social connectedness and a vibrant village community. LDK delivers lifestyle-for-life through a variety of age-friendly facilities, amenities and activities for both independent and non-independent residents. On-site facilities include a gym, art centre, chapel, dance room, theatre, library, several hospitality outlets, village shed, barber shop, hairdresser, medical consultation room, grocer, purpose designed meeting spaces and a dog park.
About the Role
Reporting to the Care Operations Manager, the Dementia Lead supports integrated and coordinated resident care and service requirements, including pre-entry clinical assessment, move-in support, care planning, care delivery, care evaluation, referral and accessing externally funded care (with associated documentation), coordination of support for residents and networking.
Role Responsibilities:
The Dementia Lead is required to coordinate and manage services for our residents living with dementia. This exciting permanent full-time position will work cohesively with our dedicated clinical team to provide safe and quality care for our residents living with dementia. With a passion for working with the elderly, our Dementia Lead supports and motivates others to make a positive difference to our residents’ lives, whilst upholding our values of Love Decency and Kindness.
This is a rewarding opportunity where you will get to lead and manage the operations of Cogan Cottage. You will demonstrate sound health management rationale, leadership, and education skills to support and model contemporary dementia care practice
Experience in Aged Care, Older Persons Mental Health, or an applicable allied health role should consider applying.
Main Duties/Responsibilities:
- Day to day management of care service delivery in our dementia care area.
- Manage ongoing review and assessment of residents, liaising with allied health, General Practitioners, and Nurse Practitioners.
- Manage family discussions, and multidisciplinary meetings where required.
- Be responsible for incident and complaint reporting and management.
- Engage in care planning and management of care planning for the residents in dementia care
- Prepare individualised behavior management support plans in collaboration with the unit team members and the in-reach specialists.
- Lead staff through demonstration of clinically relevant engagement and psychosocial support strategies for the person living with dementia.
About you:
- Registered Nurse with current AHPRA registration
- Demonstrated experience working in Dementia Care as a Registered Nurse
- Demonstrated experience in leading a team of care staff to achieve optimal outcomes for residents
- A leadership approach that is positive, and a calm and considered disposition.
