About the Role

We're looking for a Clinical Case Manager to join our supportive and welcoming Care team at Amberfield, to integrate and coordinate resident care and service requirements. The Clinical Case Manager (CCM) will ensure that all areas of support for residents and surrounding networks are considered and contacted when organising pre-entry clinical assessments, move-in support, care planning, delivery, and evaluations, referrals, accessing and documenting externally funded care. CCM's play a critical leadership component within the team, liaising with a wide range of internal and external stakeholders while sharing their knowledge and past experiences to build up and support fellow care staff. Role Responsibilities

  • Streamlining care for your portfolio of residents
  • Conducting timely and comprehensive clinical assessments
  • Overseeing resident support to determine precise care needs
  • Facilitating smooth transitions to LDK Homecare AP where needed
  • Welcoming new residents to the Village, introducing them to the care team and assistive technologies
  • Assisting residents in accessing externally funded care
  • Coordinating aged care assessments through ACAT/MyAgedCare
  • Navigating Commonwealth Home Care Packages and Support Programs
  • Handling Veterans' care coordination, including GP communication and documentation
  • Assessing eligibility for external care services
  • Collaborating with the Clinical Lead to meet resident goals and preferences

Qualifications/Certificates/Professional Experience

  • AHPRA registration 
  • Bachelor of Nursing
  • 2 years of experience working in a clinical setting
  • Experience in the community care sector/with home care packages
  • Experience working as a member of a multidisciplinary team
  • Aged care nursing experience (Desirable)
  • Experience in a team lead or management position (Desirable)
