LDK Seniors’ Living’s flagship village Greenway Views is located in Tuggeranong opposite the South Point shopping center. The village consists of 327 apartments, spread over five buildings and is home to nearly 400 residents over the age of 65. The village has over 6,000 square meters of communal space facilitating social connectedness. About the Role
We are currently seeking an experienced Care Partner to join our team. This is a fantastic opportunity for a Registered Nurse who is ready to take the next step into a coordination role. by combining clinical expertise with a genuine focus on person-centered care. Reporting to the Care Partner Manager, our Care Partner's work closely with the residents and/or their representative to ensure their care needs are evaluated and preferences are met all whilst assisting them to understand and access the level and types of funding support available.
As a Care Partner, you will:
- Coordinate the delivery of safe, high-quality, evidence-based care through assessments,
- Build and maintain strong relationships with residents and families.
- Connect residents to health and other services as needed, assist them to navigate when required.
- Collaborate with general practitioners, nurse practitioners and allied health professionals to generate an individualized care plan
- Liaise with the rostering team to ensure services are delivered as planned and in a timely manner.
- Ensure timely and accurate execution of all documentation in accordance with LDK Policy and
To succeed in this role, you’ll bring:
- Current AHPRA registration as a Registered Nurse.
- Experience in Home Care Packages, community, or aged care settings
- Strong communication skills, with a hands-on approach
- Excellent time management and problem-solving abilities
- 3-5 years of relevant nursing experience
