About the Opportunity

Partners in Recovery (PIR) supports people experiencing severe and persistent mental illness through the coordination of local health, clinical, community and social services.

MDS is seeking to employ a Support Facilitator for our Partners in Recovery (PIR) Team. This role will support each client to develop a PIR action plan based on their individual needs using a recovery oriented approach.

Key Responsibilities for this position include:

  • Commitment to principles and practices of recovery in all dealings with clients, carers and stakeholders
  • Undertake care coordination activities for referred clients
  • Undertake initial assessments of client's service and support needs
  • Develop, monitor and regularly review the client's Partners in Recovery Action Plan

The successful applicant will have:

  • Appropriate qualifications in a relevant discipline (e.g., social work, social welfare, human services) at a Cert IV or higher and/or demonstrated extensive experience
  • Knowledge of NDIS in the context of supporting individuals with psychosocial disabilty
  • A thorough understanding of and commitment to recovery-oriented principles in mental healthcare;
  • Demonstrated skills in working with consumers with complex needs
  • Strong capacity to work with challenging issues, at both the client and service delivery levels
  • Current Drivers Licence and access to a comprehensively insured motor vehicle.

The Position is on a full time flexi-time contract. This role is covered under the SCHCADS Award at a level 5 classification. Also offered is salary packaging, flexible working hours, mobile phone and leave loading.
