About the Position

Townsville Community Services are key in providing care and support to a range of clients, including Aged Care and Disability services, throughout the local region.  With a committed management team offering supervision and support, this fantastic group of professionals is looking for a full time Care Coordinator to join our team.

An experienced Registered Nurse, the Care Coordinator is accountable at an advanced practice level for the coordination, monitoring and delivery of community aged and disability support services. The Care Coordinator is also responsible for clinical staff management and leadership.

The role assists in budget management and is responsible for implementing agreed quality improvement plans for safe, client-centric care and support.

  

About You

With an extensive background in Community Aged Care or Disability  clinical management, your skills, experience and attributes will include:

  • Management and leadership including education and continuous improvement practices
  • Current hands-on nursing experience and/or the capacity to proactively refresh that skill-set in a short timeframe
  • Working within client-centred care frameworks including demonstrated experience in evaluation of client needs within the community
  • Dedicated to leading by example with a practical approach to clinical service delivery (approximately 30% of role will be hands on)
  • Relevant experience at both strategic and operational levels, preferably within a non-profit or community setting
  • Enthusiastic and compassionate attitude and a drive to succeed
  • Exceptional interpersonal and communication skills with the ability to relate effectively with internal and external stakeholders
  • Working knowledge of Microsoft Office Suite; funding and budget principles
  • Previous experience working in remote and rural locations and with Indigenous populations

Along with your practical experience, you will also bring:

  • A tertiary qualification in Nursing, with current APHRA registration
  • Relevant experience in a Senior Management position in Community Aged and/or Disability Care
  • Current Blue Card (Working with Children)
  • Current Yellow Card (Disability Services)
  • Immunisation against Tetanus, Hepatitis A and/or Hepatitis B
  • A current National Police Certificate
  • A current Queensland Drivers Licence
  • Ability to meet the physical requirements of the role
