About the Opportunity
The Village Retirement Group now has a newly created opportunity for an experienced Home Care Coordinator to join their team on a full-time basis. As this role will be working across the Yeronga, Coorparoo and Redcliffe Villages, the successful candidate will have the option to choose their preferred base at either one of these three Villages, or at the Head Office in Fortitude Valley, with the expectation that regular travel will be required between the three sites.
Reporting to the CEO, the Home Care Coordinator is responsible for coordinating effective and efficient day-to-day client services, ensuring delivery of flexible, individualised care which maximises available community resources, support and funding options. Put simply, you are working with residents based at each of the above mentioned Village sites, to assess and case manage for government funded support and/or private services, which they can access in their home environment.
This role enjoys strong working relationships with clients, family members, external stakeholders and Village teams, in order to provide quality care in line with client goals and organisational philosophy, that is underpinned by independence, flexibility and choice.
Your day-to-day responsibilities will include, but not be limited to:
- Assessing and coordinating services for village residents in a timely and accurate manner to maximize financial viability for clients and the organisation;
- Coordinating external service providers appointments for the Villages residents ie allied health services, as required;
- Conducting information sessions, morning teas and other activities to educate potential clients on the service offering and generate new business opportunities;
- Ensuring scheduled visits and client records accurately reflect required and appropriate client services; and
- Holistic assessment of the non-clinical needs of clients including physical, social, emotional, safety, financial and support systems
About You
To be considered for this position you will need to have proven experience in a similar role in either a clinical or non-clinical capacity. Post-graduate qualifications in Community Services, Aged Care, Nursing or similar will be highly regarded, but are not mandatory.
Importantly, you will be an empathetic and personable individual with good interpersonal communication skills and the ability to talk in person and over the phone with both existing and potential clients, including in large group presentation format. With a strong business development focus to the role, you will also be highly motivated and driven to identify new opportunities that are mutually beneficial for your clients and The Village Retirement Group.
The ideal candidate will be highly emotionally intelligent, flexible and resilient, with the ability to work with multiple stakeholders in an ever-evolving and rapid paced environment. As this is a newly created role, you must be self-motivated with a can-do attitude and the ability to work autonomously.
This role would be well-suited to someone who is currently working as a home care coordinator who is looking for an opportunity to assist to build an exciting new program whilst making a difference to residents lives and being well supported in their professional development
Please note: A current Driver's License and your own, reliable car will be a requirement for this position. Prior to commencement you will require First Aid certification and an Australian Federal Police Check.
