We now have an exciting opportunity for a Support Planner to join our dedicated Disability Team in Lismore, NSW. This role is a full-time position.

Reporting to the Lead Support Planner, this role will see you collaborating with clients and their care partners/stakeholders to achieve individual goals and outcomes related to the NDIS

More specifically, some of your key duties will include (but not be limited to):

  • Working with clients with complex care needs and people experiencing conflict;
  • Advocating on behalf of individual clients and groups;
  • Recruiting, orientating and inducting Community Care Workers, and providing them with professional support, supervision and training; and
  • Developing, implementing and evaluating flexible, innovative and creative person centred programs for people with disabilities, including children and young people.

To be successful in this position you will have:

  • Tertiary qualifications in Community Services, or similar;
  • At least some demonstrated experience (lived or career) in providing aged care and/or disability support services; 
  • Ideally have some experience with/knowledge of the NDIS;
  • Strong people management skills and the ability to coach and mentor others;
  • Exceptional written and verbal communication skills;
  • Computer literacy in a Windows environment is also required, particularly in word processing, spreadsheets, desktop publishing and the use of databases;
  • Have a reliable motor vehicle (including valid registration and full comprehensive third party insurance);
  • Hold a current First Aid Certificate;
  • Hold or obtain a Working with Children Check; and
  • A National Criminal History Check.
