A Rewarding Career Opportunity
Sunrise Health Service is currently looking for a full-time Social Worker to join their dedicated, multidisciplinary team in Katherine, NT.
Reporting to the Population Health Outreach Manager and working closely with the Mental Health Nurse, you would be responsible for promoting the social and emotional well-being of clients, their families, and Sunrise's remote-based staff, including activities for the prevention of self-harm or suicide, and the early detection of mental illness.
Your primary responsibilities will include, but will not be limited to:
- Promoting mental health awareness and social/emotional well-being by providing information and education to individuals and groups;
- Screening community members for the early detection of mental health issues, referring at-risk patients to specialised services;
- Following up with at-risk patients and those referred back after being discharged from specialised services;
- Assisting with case management, coordination and decision-making for clients requiring mental health care;
- Implementing and promoting evidence-based standards and policies to comply with relevant professional, industrial or legislative requirements;
- Facilitating better access to quality care for people with mental health related conditions; and
- Capturing and sharing clinical information with relevant healthcare providers, including using patient records and relevant data reporting.
What will you need to succeed?
To be successful in this position you will be a qualified Social Worker with demonstrated experience in delivering mental health care to Aboriginal communities, preferably in a remote Australian setting.
You will have an understanding of the principles of primary health care as they apply to remote locations, and be committed to the objectives and ethos of Aboriginal Community Controlled Organisations such as Sunrise. With a clear respect for Aboriginal culture, you will be able to practice in a sensitive and culturally safe manner.
Importantly, you'll be a strong team player and able to work effectively as part of a multidisciplinary team, with outstanding communication and interpersonal skills, and the ability to adapt your communication style to work effectively with people from a wide range of cultures and backgrounds.
Please note: You must be willing to undergo a National Police Check and Working with Children Check, prior to commencing, and hold an unrestricted driver’s licence.
