Who We Are:

Aspire4Life is a not-for-profit human services organisation made up of passionate people providing bespoke care and support solutions. We provide planning, assessment, and support linkage services that are delivered with the individual client's unique needs in mind.

We are implementing an essential community outreach program aimed to assist vulnerable people within Western Sydney Regions to access pathways for support.

The Opportunity:

Working predominantly in the Blacktown and Hills Shire regions, you will be providing specialist and intensive assistance to support people to understand and access aged care services in their community.

This will involve proactively engaging with people from Culturally and Linguistically Diverse (CALD) backgrounds. You'll also employ a person-centric approach to your work, ensuring the unique and, at times, complex needs of each individual have been taken into account. 

As our Care Finder program evolves, you will also have the opportunity to shape the future of this service. You’ll be pioneering the program and, as such, will have the opportunity to drive change within our organisation and throughout the vibrant communities you’ll be serving. 

Responsibilities Include:

  • The key purpose of this position is to lead and support the Care Finder navigators in their work in the community. 
  • Proactively engage with members of the target population who have one or more reasons for requiring intensive support services
  • Meeting with service providers to arrange the provision of services and to gain an understanding of service agreements
  • Delivering quality care finder support, with a focus on information, support, representation and outreach

The Requirements:

Are you an experienced aged care professional? Or maybe you’re a registered nurse looking to make a difference out in your community. We want to hear from you!

We’re after compassionate, flexible, and highly skilled care professionals who are looking to make a difference in their communities.

We’re open to individuals from a wide range of industries, but you’ll ideally be someone with a tertiary qualification in one of the following disciplines (or similar):

  • Social work (when combined with relevant experience in performing assessments)
  • Nursing
  • Medical practitioner
  • Occupational therapy
  • Physiotherapy 

Ideally, you’ll have over 3 years of experience in a related field, with some aged or community care exposure. To succeed in this role, you should ideally also have AHPRA registration and the demonstrated ability to work in a clinical environment.

Most importantly, you will be an empathetic and agile professional who is passionate about leading a team to provide high-quality, person-centred Aged Care navigation support.

Clinicians with experience in a community care setting or within a service provider organisation will be highly regarded, as will Social Workers with relevant industry experience performing assessments in a clinical setting.
