Aspire4Life is a not-for-profit human services organisation made up of passionate people providing bespoke care and support solutions. We provide planning, assessment, and support linkage services that are delivered with the individual client's unique needs in mind. We partner with government organisations and other like-minded leaders in the community to deliver a number of vital services, including Aged Care Regional Assessment Services. 

We now have rewarding opportunities available for passionate Home Support Assessors to join our team in the South East Sydney and North Sydney care planning regions on either a full-time or part-time basis.

The Opportunity:

As our next Home Support Assessor, you’ll be undertaking a holistic assessment style, with the overarching goal of supporting clients in continuing to live healthy and independent lives in their chosen community.

You’ll have the opportunity to visit clients in their homes throughout the South East Sydney or North Sydney care planning regions to complete assessments, depending on your location. Although you will be on the road for much of the time, this role will also afford you the flexibility to complete your administrative work from home, your local cafe, or wherever else you feel most productive.

Our dedicated scheduling team will ensure that your day runs smoothly, organising your schedule and allocating resources accordingly to give you the tools you need to work autonomously and effectively.

Responsibilities Include:

  • Undertaking assessment activities as scheduled to ensure timeframes and response times are compliant with operational requirements
  • Participating in Community Engagement Events in order to increase awareness of the Regional Assessment Services and associated services across the region
  • Maintaining and developing comprehensive knowledge of formal and informal regional service providers, service options, and support agencies

The Requirements:

We’re looking for experienced professionals from the Aged Care, Nursing, Occupational Therapy, Physiotherapy or Social Work sectors to join our team and begin a rewarding career with Aspire 4 Life. 

To succeed in this role, you should have a minimum Certificate IV in Aged Care or Community Services, however, a Bachelor’s Degree in a related discipline will be highly regarded.

You’ll also need previous experience in a similar role, where you were responsible for assessment and planning of services in aged care or a similar industry e.g. with the NDIS.

Other requirements include:

  • An unrestricted driver’s license & your own vehicle
  • Ability to confidently use technology such as smartphones and laptops

As our next Home Support Assessor, you will enjoy a competitive salary of circa $63,353 - $80,016 + superannuation.
