About Partners in Recovery (PIR)
The PIR (Partners in Recovery) initiative is the collaborative effort of The Sunshine Coast Medicare Local in partnership with other service providers. The ultimate objective of the initiative is to improve the system response to, and outcomes for people living with severe and persistent mental illness.
Partners in Recovery aims to better support people with a lived experience of severe and persistent mental illness with complex needs, and their carers and families, by working in a more collaborative, coordinated, and integrated way with services the person may need. PIR will facilitate streamlined access to the clinical and other service and support needs of people experiencing severe and persistent mental illness with complex needs, and support improved coordination of multi-agency response. About the Opportunities - 8 Support Facilitator positions available
To establish the Partners in Recovery (PIR) initiative, the partnership is seeking eight (8) Support Facilitators in the Sunshine Coast and Gympie region.
As a Support Facilitator, you will be responsible for the facilitation of services and supports for people who are eligible for PIR services.
Your duties will include (but will not be limited to)
- Engaging in productive and respectful relationships with people living with serious mental illness, their family and networks
- Regularly consulting with people to develop and review their PIR action plan
- Supporting local area development activities such as promotional, networking and communication opportunities
- Contributing to service mapping and gap analysis in your area and
- Ensuring effective, recovery focused wrap around services for people by developing strategic partnerships with key stakeholders
Your success will be maximised by your knowledge of the local health and community sector along with experience working with clinical/health, welfare, support, housing and employment services.
The Partners in Recovery initiative on the Sunshine Coast and Gympie areas is seeking people who:
- Understand and demonstrate skills in working alongside people living with severe and persistent mental illness who have complex needs and who have experienced difficulty engaging with services,
- Actively collaborate with people living with mental illness, their families and networks to seek innovative solutions to complex problems using your resourcefulness and initiative to enhance opportunities for recovery,
- Have solid experience in working within a multidisciplinary service system,
- Have highly developed negotiation and networking skills along with an ability to think strategically,
- Are proactive, organised, and can work both autonomously and be part of the team,
- Can develop client centred action plans, maintain progress notes and write reports,
- Recognise systemic issues and barriers and have capacity to apply your analytic and problem solving skills,
- Display a high level of perseverance, resilience and self care,
- Are committed to ongoing professional development and can work within a recovery framework.

They are a well-known and respected, medium-sized, non-government, not-for-profit community managed organisation incorporated in May 1999. Choice delivers a range of centre-based activities and individual recovery focused support services to adults with symptoms of mental illness and/or psychiatric disability and bases its model of service delivery on the recovery paradigm. Sunshine Coast Medicare Local’s purpose includes identifying primary healthcare needs in the region, establishing priorities in a region-wide primary health plan, and commissioning local healthcare providers to fulfil these needs and priorities. Their priorities include integrating health services to improve the patient journey between services, and promoting and demonstrating best practice in all areas of service. Sunshine Coast Medicare Local is also a Host Agency and will employ a ‘floating’ Support Facilitator.
