About the Organisation
The Kelvale Medical Group is a purpose-built General Practice located some 20km southeast of Perth at Kelmscott, WA. The Practice has modern facilities for the management of a large range of medical conditions. All manner of GP procedures can be competently carried out in this well equipped surgery including excisions, vasectomy, ECG, lung function testing and audiometry.

About the Opportunity
As part of its current growth, Kelvale Medical Services has an exciting opportunity for a permanent Registered Nurse (Chronic Disease Management) to join their busy team in Kelmscott, WA.
In this role you will be responsible for providing clinical management for complex patients with cardiac, respiratory, diabetes and other chronic conditions including assessment, education and self management. Using best practice, you will create effective care plans, with a focus on self management to promote optimum outcomes.
You will have the flexibility of choosing either part-time work (28 hours per week, in Chronic Disease) OR full-time work (in which case you would spend half of your time in Chronic Disease and the other half working as an RN in a different discipline of your choice).
To be considered you must be registered with AHPRA, with previous experience in General Practice highly desirable. Candidates with recent clinical experience and an interest in chronic disease programs will be highly regarded.
Kelvale Medical Group are seeking a highly motivated team player, with well developed organisational and interpersonal skills and a high attention to detail. You will have excellent written and oral communication skills and the ability to develop rapport with a wide range of patients and stakeholders. Importantly, you will demonstrate the ability to work both independently as well as within an interdisciplinary team environment to provide outcome based care.
