About the Opportunity
Australian Multicultural Community Services Inc.(AMCS) has a fantastic opportunity for a Community Care Manager to join their team based in Maidstone, VIC.
Reporting to the Chief Executive Officer, you will be responsible for managing the Home Care Packages program, the Brokerage Service and Disability Services. Additionally, you will be expected to proactively contribute to the overall agency functioning and performance.
This is a great opportunity to lead a small team of case managers working together as part of a community network to assist consumers to maintain an active and independent lifestyle.
More specifically, some of your key duties and responsibilities will include:
- Ensuring compliance with relevant program standards and legislation;
- Managing the programs budget to enable continued funding and service delivery;
- Monitoring and evaluating the programs in your area of responsibility;
- Ensuring that all reporting requirements are met;
- Conducting and participating in regular case management team meetings; and
- Managing and providing leadership to the HCP team.
To be successful in this position, you will need a tertiary degree in Management or another relevant discipline coupled with strong experience in management, staff supervision and team leadership.
You will have experience in the planning, coordination and delivery of services to the frail and aged as well as people with a disability living in their own homes. In addition, you will need strong skills in case management, assessment, care planning and coordination as well as conflict management.
Importantly, you will have strong knowledge and understanding of the Aged Care Act (1997) and Consumer Directed Care and the ability to maintain confidentiality and discretion when dealing with people.
