About the Organisation
The Wheatbelt GP Network is a NFP regional leader in primary health care delivery across the Wheatbelt region of WA. The Network is a progressive service provider that works with health professionals, hospitals, three levels of government, local agencies and community members to deliver integrated, evidence based, multidisciplinary primary healthcare.
The Network supplies GP, nursing, physiotherapy, dietetics, diabetic education, Aboriginal health, counselling, psychology, ultrasound, pathology collection and support services. Services are delivered from 5 sites, as well as through outreach services. The Wheatbelt Health Centre (WHC) is a modern facility built in partnership with the Commonwealth under the super clinics program and is the flag ship facility for the Network. Whilst there may be work across a range of locations the majority of the work will focus on areas that are hard to reach and maybe anywhere in the Wheatbelt region.
About the Opportunity
The Wheatbelt GP Network has multiple opportunities for Credentialed Diabetes Educators to join their multidisciplinary team based in Northam, WA, on a full-time basis.
Reporting to the Patient Services Manager, you will be responsible for promoting optimal health and wellbeing for individuals and communities, providing high-quality diabetes education services to those living with diabetes and their families across the Wheatbelt region.
More specifically, your duties will include:
- Leading group diabetes education sessions;
- Monitoring patient medication, diet, nutrition, and blood glucose levels, while also instructing on techniques for self-monitoring and injections;
- Liaising with key stakeholders to maintain an integrated approach to the management of diabetes;
- Collaborating with local GPs, hospitals, primary health care providers, and Aboriginal health services, to ensure that clinical pathways and referral protocols are developed in accordance with contemporary clinical management guidelines; and
- Developing an evaluation plan, measuring quantitative and qualitative data for clinical outcomes through defined data sets, case studies, and client surveys.
The Wheatbelt GP Network are seeking candidates committed to continuing their professional development, who are passionate about health promotion, and motivated to deliver the best service possible.
Successful applicants will hold a Credentialed Diabetes Educator (CDE) qualification recognised by the ADEA, with previous experience delivering CDE services to clients in rural and remote areas (preferably in a primary care environment).
You will need to demonstrate excellent time management and organisational skills, with a high level of computer literacy using Microsoft Office Suite and Patient Information Systems. It is also essential that you work effectively as a member of a multidisciplinary team, with interpersonal skill as well as written communication skill.
Please note: To be eligible for this role, you must hold a current C Class Driver’s License, and have current Police Clearance.
