Who are we?

Wintringham is a not-for-profit organisation providing an extensive range of aged care, housing and related support services to clients aged 50-plus who have been homeless or at risk of homelessness. We employ over 900 staff and operate from multiple locations across Melbourne, Hobart and regional Victoria. Our team of Carers, Case Managers and Outreach Workers provide holistic care and support to clients who live independently in their own homes.

 

Your new role

Wintringham is seeking a motivated and enthusiastic person with a commitment to social justice to join our multi-disciplinary community team as a Part Time After Hours Case Manager. As the After Hours Case Manager, you will be responsible for ensuring all community clients are able to access telephone support after hours between 1700-0900 Monday to Friday, between 1700 Friday and 0900 Monday and from 1700 the last working day before any Public Holiday/s to 0900 on the next working day. Holiday/s to 0900 on the next working day. This role will require a candidate who can assess a situation remotely, address immediate issues, use the escalation process appropriately, and provide a debriefing after an incident.

 

*When re-called to duty (not return to the workplace) you will receive a minimum of 1 hour of over time. Over time is paid at time and a half for the first two hours and double time for anything over 2 hours. Overtime is calculated based on your ordinary rate of pay. You will receive a one off on-call allowance of $53.988 for each shift you are rostered for on-call.

 

 

Your responsibilities and duties will include:

  • Ensuring all phone calls received on the afterhours phone are answered in a timely manner
  • Management of afterhours sick leave roster for staff working in the community after hours.
  • Utilising a riskbased management approach when addressing each individual request and taking the appropriate action based on the severity of the request
  • Document on the After Hours Service Log Form and enter notes under the relevant client in GoldCare.
  • Have a thorough understanding of the escalation process. Supervise and provide leadership to community care staff working after hours.

 

We are looking for candidates with the following:

  • An approved tertiary qualification in Nursing, Social Work, Allied Health or Welfare
  • Demonstrated experience in risk assessment and case management.
  • Preference will be given to current Community Case Managers
  • Part time employees encouraged to apply.
  • A non-judgmental approach with the ability to work with people from varying socio-economic backgrounds

Successful candidates will be required to show evidence of having three doses of a COVID-19 vaccine as well as a 2022 influenza vaccine prior to commencing employment with Wintringham.
