Wintringham’s Supportive Housing Project – the Audrey Rainsford Building is comprised of beautifully finished apartments centrally located in Carlton. The Supportive Housing multi-disciplinary team consists of  Coordinators, Case Workers, Recreation staff and Tenancy staff, who are in place to support a staged approach to independent living and enhance the community engagement of its residents.

 

Your new role

Two exciting opportunities exist to be an integral part of a core project at Wintringham. The HHS - Supportive Housing Case Worker positions are suitable for candidates with experience in Intensive Case Management or skills in AOD Mental Health, Family Violence or Homelessness. These roles will manage a caseload of residents at Audrey Rainsford, helping each individual to identify their strengths, as well as their support needs, to develop individualized case plans. The positions will work collaboratively with residents to create a meaningful life, assisting them to develop self-management skills across mental and physical wellbeing, personal safety, and crisis management and harm minimization with the aim of supporting them through a staged approach to living independently and community engagement.

The Case worker roles are Full Time, each working a rotating roster across Monday to Friday, including varying weekday shifts from 8:30am-9:30pm

 

Your responsibilities and duties will also include:

  • Work with residents by linking and referring them into clinical services as well as social and recreational activities.
  • Utilise your knowledge of harm reduction and brief intervention, trauma informed practice, conflict resolution, MARAM and Housing First principals to manage a case load of clients.
  • Ensure residents obtain access to Commonwealth Home Support Program (CHSP), Home Care Packages and NDIS services by providing referrals and case management services.
  • Responding to resident feedback and complaints in line with Wintringham policy and procedures.
  • Utilising relationships within sectors to assist access into other services and systems.
  • Working collaboratively with the other support workers and teams to ensure a collaborative team culture is maintained.

 

We are looking for candidates with the following:

  • A qualification in Social Work, Welfare, Community Services or equivalent.
  • Experience in a support worker role in Intensive Case Management with specific training and/or experience in: AOD Mental Health and Family Violence or Homelessness services.
  • An understanding of the Housing and Homelessness Service sector My Aged Care, NDIS and other relevant service systems.
  • Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support.
  • Well-developed communications skills and the ability to with engage people from varying backgrounds.
  • First Aid Certificate.
