  • Competitive base rate - $39.94 – $49.47 per hour + 17.5% leave loading
  • Generous Motor Vehicle Allowance - $8.65/hr
  • Fixed Term Position – 6 months duration
  • Part Time role – 22.8 hours weekly (3 days)
  • Based at our Bendigo Office and servicing the Bendigo region (VIC)

Your new role

Wintringham is seeking a dynamic and proactive Case Manager, to join our multi-disciplinary Support at Home team on a part time basis. This dynamic opportunity will work with our in-house rostering team to support clients , taking into account their goals, interests and preferences to help them make informed choices about the services and assistance they’d like to receive, while adhering to their budget.

This flexible role will be required to manage a small case load of clients covering Bendigo and surrounding suburbs.

The position will cover 22.8 hours per week with the workdays being negotiable based on your availability (Monday to Friday) and will run for 6 months.

 

Your duties and responsibilities will include:

  • Assisting clients living in their own homes to maintain their independence and lifestyle of their choice by providing a high level of care
  • Helping address issues of social isolation for program clients and assist clients to maintain or improve their accommodation.
  • Undertaking initial and ongoing assessments for new clients.
  • Developing and implementing care plans and clients’ schedules.
  • Participate in care reviews with managers, clients and carers
  • Effectively managing services provided within budgetary constraints
  • Working with an in-house rostering team to ensure clients receive the care and supports they require according to their funded level of package

 

We are looking for candidates with the following:

  • An approved tertiary qualification in Social Work, Allied Health or Nursing.
  • Experience working in mental health(desirable)
  • Previous experience as a Case Manager (would be highly regarded)
  • High level computer, documentation, and budgeting skills (mandatory)
  • Current drivers’ licence with a reliable vehicle suitable for transporting clients
  • Knowledge of housing and support issues facing older homeless people whom may have a physical and/or mental disability.
  • Skills in assessing clients’ psychosocial, care, support, health and housing needs.
