About the role
Reporting to the Clinical Manager Ward, the Discharge Planner plays an important role in coordinating safe, timely, and effective discharge plans for patients transitioning from hospital to home or other care settings. This role involves close collaboration with multidisciplinary teams, patients, and families to ensure discharge goals are met, support services are organised, and continuity of care is maintained.
This is an RN Level 2 Year 1 position, available on a part-time basis (0.5 or 0.6 FTE).

About you
You will be a clinically proficient and appropriately experienced Registered Nurse who is motivated and passionate about providing exceptional care throughout the patient journey.
This position requires an individual who has:
- Strong knowledge of acute care nursing principles and preferably experience in patient care coordination or discharge planning.
- Knowledge of surgical procedures, monitoring and equipment, pain management methods and medications.
- Excellent communication and interpersonal skills with the ability to engage professionally and respectfully with patients and all members of the multidisciplinary team as well as external providers.
- Ability to work under pressure and meet deadlines.
- Strong organisational skills and attention to detail with excellent assessment ability and critical thinking mindset.
- Proficiency in relevant systems, particularly Microsoft Office Suite and Patient Information Management System (PiMS).
- Demonstrated ability to anticipate and identify potential problems quickly and efficiently, adapt, and communicate these to stakeholders across the continuum of care to ensure safe, positive patient outcomes.
- The highest ethical standards and treats all hospital and patient matters with confidentiality and respect.
