About Blue Haven:

Blue Haven Illawarra is owned and operated by Kiama Municipal Council. We offer a true continuum of aged care services and a wide range of services for people in the Illawarra region. We provide aged care, in-home support, and coordination, community transport and independent living.

Blue Haven require passionate, qualified, and experienced professionals to join our organisation to support our aged care clients and residents. Blue Haven Bonaira services over 1,200 clients across Residential Aged Care, Retirement Living, Home Care Packages, Commonwealth Home Support Program and Community Transport across the Illawarra and Shoalhaven.

The role:

We are looking to recruit a Case Manager to:

  • Assist in the coordination of the Commonwealth Home Support Program (CHSP), Community Transport and Home Case Packages auspice by Kiama Council and operating as Blue Haven, achieve the objectives of the projects as determined by the government funding guidelines
  • Deliver services to the clients of Blue Haven Community in an ethical and professional manner with regard to legislative direction and service guidelines
  • Participate as an active member of a multi-disciplinary Community Services team. This position may have the opportunity to develop skills working across Blue Haven.

To be successful in this role you will need:

  • Certificate IV, Diploma or Degree in Community Services or Equivalent Health Qualifications or working towards
  • Experience caring for older people and their carer's
  • Knowledge of an enablement and/or person-centered approach to client assessment and case management processes
  • Experience working as a team member or with minimal supervision
  • Class C Driver’s Licence.

We offer:

  • Permanent full time, 35 hours per week with a possibility for job share arrangement
  • Salary from $63,968.32 to $90,961.00 pro rata (depending on qualifications, skills and experience)  
  • Plus 11% superannuation and applicable penalty rates
