We are recruiting two Registered Nurses – Clinical Care Partners to provide advanced clinical care, leadership, and care coordination across our community programs. This is a hybrid role, based in Campbelltown or Wollongong, with regular work‑from‑home days and occasional travel between locations.

Role highlights

  • $110,000 gross per year ($55.68/hr) + up to $18,550 tax‑free NFP salary packaging
  • Hybrid work model – Campbelltown or Wollongong + WFH each week

What you’ll be doing

  • Provide clinical leadership, guidance, and mentoring to nurses and care partners
  • Manage a caseload of clients with complex clinical needs
  • Coordinate multidisciplinary care and support client transitions
  • Advocate for clients and support person‑centred care planning
  • Drive quality improvement, compliance, and clinical governance
  • Build strong relationships with clients, families, and health professionals

What we’re looking for

Essential

  • Registered Nurse with current AHPRA registration
  • Post‑graduate experience in community, aged care, or primary health
  • Demonstrated clinical leadership and mentoring capability
  • Current NSW Driver’s Licence
  • Current First Aid Certificate

Desirable (highly regarded)

  • Advanced clinical assessment and case management experience
  • Knowledge of Home Care Packages (HCP) and Support at Home program
  • Experience in palliative care, dementia, or end‑of‑life care
  • Understanding of Aged Care Quality Standards and clinical governance
