We are recruiting a Registered Nurse who will work in a small, collaborative clinical team to deliver high-quality clinical care and care coordination across our community programs. This role will be based in the Illawarra region, with occasional travel to the Shoalhaven region required.

  • Payrate: $50.68/hour + NFP Salary Packaging + KM reimbursement 
  • Hours per week: Full time, 38 hours/ week. We are open to a part-time role for the right candidate. 
  • Position Type: Permanent
  • Location: Illawarra, with occasional travel to the Shoalhaven region

What you’ll be doing

  • Deliver direct, comprehensive nursing care and individual case management to aged care and NDIS participants in their homes and community
  • Conduct nursing assessments and develop, implement and review car plans and Advanced Health care plans in collaboration with clients and their families/ carers.
  • Support direct care workers through education and guidance to maintain best practice clinical standards
  • Coordinate services across health providers and community stakeholders to meet participants changing needs.

 

What we’re looking for

Essential

  • Registered Nurse with current AHPRA registration
  • Demonstrated experience working with complex clients in aged care, disability, or community settings 
  • Demonstrated clinical leadership and mentoring capability
  • Current NSW Driver’s Licence
  • Current First Aid Certificate

Desirable (highly regarded)

  • Advanced clinical assessment and case management experience
  • Knowledge of Home Care Packages (HCP) and Support at Home program
  • Experience in palliative care, dementia, or end‑of‑life care
  • Understanding of Aged Care Quality Standards and clinical governance
