As a Case Manager, you will play a crucial role in within our Permanency Support Program (PSP) Team in assessing client needs, developing personalised care plans, and coordinating services to support their journey toward stability and well-being. Your expertise will guide individuals and families through challenging times, providing them with the resources and support they need to succeed.

- Hourly Pay Rate: $46.70 to $50.25 per hour (depending on experience)
- Hours per Week: 38

What you bring to the role
This position requires a compassionate approach, strong communication skills, resilience and the ability to work collaboratively with various stakeholders. You will be the advocate for your clients, ensuring they receive the best possible care and support tailored to their unique situations. We are seeking a motivated and empathetic individual who is passionate about helping others and making a positive impact in the community.

- Relevant tertiary qualifications in Psychology, Social Work, Welfare, Behavioural or Social Sciences or other relevant disciplines.
- Proven experience in case management, with a focus on Out of Home Care (OOHC) and permanency planning.
- Strong organisational and time-management skills.
- Exceptional interpersonal and communication abilities.
- Ability to work independently and as part of a team.
- Paid NSW Working with Children Check (WWCC) and willingness to undergo a National Criminal History Check (NCHRC).
