What You Bring to the Role

As a Case Manager, you will play a crucial role in assessing client needs, developing personalized care plans, and coordinating services to support their journey toward stability and well-being. Your expertise will guide individuals and families through challenging times, providing them with the resources and support they need to succeed.

This position requires a compassionate approach, strong communication skills, and the ability to work collaboratively with various stakeholders. You will be the advocate for your clients, ensuring they receive the best possible care and support tailored to their unique situations.

We are seeking a motivated and empathetic individual who is passionate about helping others and making a positive impact in the community.

  • Relevant tertiary qualifications in Psychology, Social Work, Welfare, Behavioural or Social Sciences or other relevant disciplines
  • Proven experience in case management or a related field.
  • Strong organizational and time-management skills.
  • Exceptional interpersonal and communication abilities.
  • Ability to work independently and as part of a team.
