About the Opportunity
Our Care Coordination space is growing. To support this growth whilst maintaining our exceptional service delivery, we are offering opportunities for Home Support Team Members to step into a Care Coordination Support Officer role as a fixed-term secondment.
As a part of this secondment, you will provide administrative and co-ordination support to the Care Coordination team as well as managing a client case load, alongside your care and support to clients in their home. You will ensure clients are receiving accurate information allowing them to make individual choices while feeling heard, considered, and respected.
This role works collaboratively within a fully integrated multi-disciplinary environment being knowledgeable in ECH services and programs, which will enable you to identify which areas may be beneficial to clients and their individual needs. You will coordinate services to ensure continuity of care as clients transfer between different levels of care.
You will receive training and support to help you to succeed in this role.
