THE POSITION The Activity Support Facilitator will support consumer/ participant access and participation in activities of their choice, facilitated by Council’s Life Choices – Support Services staff at numerous locations.
Reporting to Coordinator Activity and Care Services, you will:
Maintain awareness of policies and funding guidelines, implement them accurately, and develop tailored support plans.
Assist the Coordinator of Activity and Care services in addressing access barriers for specialised consumer / participant groups.
Assist new consumers /participants and maintains strict confidentiality.
Organise engaging activities, support consumers / participants during planned events, and facilitate their participation in various activities.
Conduct risk assessments to ensure safety, adhere to health and safety regulations, and maintain awareness of relevant legislation.
Uphold health and safety standards applicable to both consumers / participants and program activities.
Maintain updated records, ensuring quality documentation and adherence to food preparation legislation.
Participate in training for continuous improvement, document incidents promptly, and support administrative duties.
Engage in discussions, meetings, and workshops, contributing to program enhancement and continual quality improvement.
Manage electronic databases, statistics, and the distribution of program awareness material.
Engage in back up support for other teams within LC-SS
WHO WE ARE LOOKING FOR Specifically, you will have:
Essential
Certificate III or IV in Community Services (Aged Care Work, Disability Work and/or Community Work) or an equivalent qualification
Current Class C (Car) Drivers License
Ability to meet the requirements for a Criminal Record and Police Check, NDIS Worker Screening Certificate and NSW Working with Children Check
Desirable
Current First Aid Certificate
Class LR Drivers License
