About the Role
As a Clinical Nurse, you will deliver safe, evidence‑based clinical care aligned with the Aged Care Quality Standards, NSW Health requirements and Council policies. You will play a key role in clinical governance, risk management and continuous improvement across Council’s aged and community care programs.
Key Responsibilities
Deliver clinical assessments, care plans, wound care, medication support and chronic disease monitoring
Identify and escalate clinical deterioration; maintain accurate clinical documentation
Work collaboratively with Participants, families, carers and multidisciplinary teams
Promote wellbeing, independence and culturally safe, person‑centred care
Manage clinical risks, incidents, infection control and safe work practices
Provide clinical guidance, mentoring and training to support workers and new staff
Liaise with GPs, allied health, hospitals and external providers to coordinate care
Maintain accurate records, prepare reports and contribute to service planning and improvement
About You
Essential
Current AHPRA registration (Registered Nurse – Division 1)
Clinical experience in aged care, community nursing or chronic disease management
Strong assessment and care‑planning capability
Understanding of Aged Care Quality Standards and clinical governance
High‑level communication, documentation and digital literacy
Ability to work collaboratively with Participants, families and multidisciplinary teams
Current NSW Driver Licence, National Police Check and WWCC
Desirable
Postgraduate qualifications in Aged Care, Community Health or Chronic Disease
Experience in local government or community‑based service delivery
Knowledge of My Aged Care and the Support at Home Program
Experience delivering staff or clinical training
