Your new role

We are currently seeking a flexible and enthusiastic HHS Support/Case Worker to join our team at Delahey Independent Living. We are looking for candidates with strong communication and engagement skills who can build rapport with older people experiencing homelessness, housing insecurity, social isolation, and other complex challenges.

HHS Support/Case Workers provide person-centred support, case management, and advocacy to help vulnerable older people maintain their housing, remain independent, improve their wellbeing, and access the services they need. All service delivery is guided by Wintringham's values, mission, and commitment to social justice.

Your responsibilities and duties will include:

• Provide person-centred case management and support using evidence-based approaches such as Harm Reduction, Motivational Interviewing, Trauma-Informed Practice, MARAM, Conflict Resolution, and Housing First principles.
• Work collaboratively with clients to develop individual support plans, build independence, strengthen daily living and self-management skills, and support recovery, wellbeing, and personal safety.
• Facilitate access to housing, health, community, aged care, CHSP, Home Care Package, NDIS, and other support services through advocacy, referrals, and service coordination.
• Support clients to maintain their tenancies, increase community engagement, strengthen natural support networks, and reduce the impact of social isolation.
• Maintain accurate client records, respond to feedback and complaints, ensure privacy and duty of care obligations are met, and contribute to high-quality, culturally safe service delivery.

We are looking for candidates with the following:

• A tertiary qualification in Social Work, Welfare, Community Services or a related discipline, or extensive experience working within the homelessness or community services sector
• Experience in intensive case management, homelessness, housing, mental health, AOD, family violence, or related community service settings, with a commitment to best-practice service delivery.
• Strong understanding of the housing and homelessness sector, including My Aged Care, NDIS, and relevant government policies, standards, and compliance requirements.
• Excellent communication and engagement skills, with the ability to work effectively with diverse communities and uphold social justice and client-centred practice principles.
• First Aid Level 2
• Current Driver’s Licence

• Competitive pay rates: $42.63 - $51.13 p/hr plus 25% Casual Loading and superannuation (dependent on qualifications + experience)
